In this form you can select the grade playing, what gear you require and a payment method.

Payment options are: 

1) Pay to team managers or at the ClubBar

2) Internet Banking

3) Setup an AP

4) Pay by Credit Card (incurs a $3 processing fee)

All players need to fill this out, non-dependent on the payment method you are choosing.
